As well as their name if they wish their site name can also be added to this one, but for the most part this plaque is the "Quote Plaque" when filling out the form and your request is this one, please include your quote! If you only want your name displayed please specifiy.
2. This plaque will have name, site name, and favorable quote, or any other tidbit information the member may wish to have. Please specifiy exactly what you wish to have on the image. Member number will be in the upper right hand corner.
3. Smaller versions, but will contain Name, Site, and Quote.
4. Larger version of the appove with a little change, Name, Site and Quote can be displayed.
